Co-op Add/Drop Form - Use this form to start cooping or stop your regular coop. Please check in with our administrative office or contact us at 650.462.8870 to arrange for adding a coop time. Arrangements for adding or dropping should be made 2 weeks in advance.
Co-op Buy Out - Use this form if you cannot coop and have not made arrangements to swap your coop with another family.
Schedule Change - If you wish to change your child's schedule, you must submit this form. Schedule changes are at the discretion of the Enrollment Coordinator, and it may not be possible to honor all requests.
Extra Hours - For part-time children, you can request extra hours of care from your classroom teaching team with the extra hours form. (Availability is limited, and additional hours must be preapproved by a teacher in your child's classroom.) Extra hours are billed at a rate of $20.00 per hour.
Exit Notice - A thirty (30) day exit notice is required for the refund of space deposits and can be applied to the last month’s bill. Refunds are returned via mailed check 10 days after your exit date. Final fees are due and payable by the last day of attendance. Please submit exit notices to enrollment@ccscparentcoop.org. If notice is given between the first and the 15th of the month, the 30 days will be counted as of the 15th. If notice is given between the 15th and the last day of the month, the 30 days will be counted as of the 1st of the following month. A family who leaves the Center on or before the 15th of the month will be billed for only half of the month, if proper 30 day exit notice is given. A family who exits after the 15th will be billed for the whole month.
Summer Exit Notice - All families exiting during the summer months (June, July or August) are required to provide an exit notice prior to May 1st. Summer leave dates will be billed either the 15th or 30th/31st of the month, based on the formal exit notice policy. Space deposits are returned via mailed check 10 days after your exit date. Final fees are due and payable by the last day of attendance. Please submit exit notices to enrollment@ccscparentcoop.org.
Incoming New Contracted Families - The exit notice policy does not apply prior to your contracted start date. Space Deposits serve as a security to CCSC to ensure enrollment. Space deposits are non-refundable if a family decides not to start the program, even before their contracted start date. Please submit exit notices to enrollment@ccscparentcoop.org.
Retraction of enrollment is not included as part of the exit notice policy. Retraction of enrollment renders space deposit non-refundable, even when an exit notice is given. The exit notice policy does not apply prior to your contracted start date.
Tuition Express Application - Use this form to have your tuition payments processed electronically. Please note that CCSC’s preferred method of payment is through electronic funds transfer. CCSC does not accept credit card payments for tuition.
